a couple of suggstions:

- be bold about what you have achieved this year.  This is not the moment to be modest

- pick up one or/no more than 2 areas that you see as opporunity to improve or work on - no one is perfect

- if you feel strong about it, like what you did,  ask for a raise directly. 

- be prepared to be rejected or to have discussions to justify

- worst thing is to over-think about this: should I do this or should not. Be reasonable but assertive with your idea.
