Once a LC get approved, DOL will mail you a

notice with several green papers which are printed in both side. All the green papers are the information which your lawyer put in the ETA 9089 form. So, when you file 140, you must attach the notice and the green papers to USCIS.
So, the eta 9089 includs many personal information,that is why you can not use it.
