"If you or any of your Dependents are eligible for subsidized medical coverage that is not COBRA coverage (“Subsidized Coverage”) from another employer or a former employer but decline that coverage, you will need to pay an additional contribution above the normal rate, if you enroll yourself or such Dependent, as applicable, in XXX. The additional contribution amount is based on your Annual Pay. You must pay the additional contribution amount for each individual (including you, if applicable) you enroll in XXX who is eligible for but declines other Subsidized Coverage from the individual’s employer or former employer; however, the additional contribution is waived for each individual (you or your Dependent) under age 19.
