1）. I-130，Part5，我两年前已为母亲申请了130，所以question1是 Yes.  

Provide name,place,date of filling: 这里是不是填母亲的姓名，我的Physical Address ？date filled 是不是母亲I130的 received date (或 proirity date) ？

我不确定place of filing应该填哪里。date filed是I-130的received date（应该跟priority date一样）。

2）. I485's Evidence of continuously maintaining Lawful status since arrival in US:

Instructions says:  Include evidence for every time you entered the United States and for the time periods spent in the United States.

我爸从2011至今入境了5次，这样我需要把过去每一次的记录都要附上？ 还是只用most recent I-94就可以？

你爸属于Immediate Relative类型。不需要提供这个。（他申请时不需要有身份、之前有没有失去过身份无所谓。）只有其他类型需要提供这个。
