'Bend over backwards' means to make an extreme, extra effort, or to go to great lengths to help, accommodate, or please someone. It signifies intense, often difficult, dedication to achieving a goal or satisfying a request. 

It is often used when providing significant assistance, such as "She bent over backwards to help him"

 It can imply going above and beyond what is expected. Sometimes it can express frustration, such as when one feels forced to do too much for someone else.

This phrase dates from the turn of the twentieth century, and while its origin is vague, it's not hard to figure out how it came to mean " Trying very hard to do something good or helpful"
