快餐店和糖尿病

今天早上看到内分泌期刊上有一个蛮有意思的文章。

一个研究收集了四百万病人的电子医疗记录。这些病人都没有糖尿病历史。他们跟踪这些病人五年半，13.2%的病人在这期间发生了糖尿病，其中老年病人和黑人病人糖尿病增加最为明显。

他们发现，任何一个区域，快餐店每增加百分之十，那个区域的糖尿病就会增加百分之一到百分之二。而多建立新鲜事物的杂货店，那个区域的糖尿病会下降。

小时候考GRE都知道怎么来辩证这样的问题，这种调研中显示快餐店和糖尿病的这个现象只能说是相关性，而不是因果性。没有办法证明快餐店的增加是直接导致糖尿病的原因，因为大部分快餐店都是在城市内部，经济条件比较差的地方，病人经济紧张的时候，会选择便宜可口，但是热量奇高的快餐，同时在医疗预防和寻求早起干预的意识上也会相对比较差，这些都会引起糖尿病发病率相对增高。

文章的作者说，这个研究希望能够提醒城市规划，减少快餐店的建立，而多鼓励新鲜杂货店的建立。这个想法，虽然很好，实验也做得很好，但是对现实，会有多大的区别呢？快餐店巨头会care么？每个月固定低收入的病人会care么？城市规划的人会care么？
